New Patient Information

 

    As new patients to Primary Health and Wellness Centers, you will be asked to provide personal information and insurance information. Please be prepared to provide this information when you schedule your appointment. If you are already an established patient and your information has changed, please let our staff know when you arrive to allow us to update your information.

 

    For your convenience, we have attached the forms you will be asked to complete and sign when you arrive for your first appointment. You may print these forms and complete them in advance if you would like. Doing so will save time and paperwork for you on the day of our appointment. Just click on the document link below to print the required information. If you prefer to complete them in the office, we ask that you arrive 30 minutes early for your appointment.    During your new patient visit please bring your insurance card, valid ID, medication list, and any copay if required by your insurance company.
